Introduction: Occupational stress among nurses working in palliative care represents an important worker health issue resulting from continuous exposure to suffering, terminal illness and intense emotional demands. Objective: To analyze factors associated with occupational stress among nurses working in palliative care, considering its repercussions on professional practice and psychophysical health. Methodology: This is an integrative literature review conducted in the SciELO, LILACS, PubMed and Google Scholar databases. The descriptors “occupational stress”, “palliative care”, “worker health”, “nurse” and “work-related suffering” were combined using the Boolean operators AND and OR. Studies published between 2021 and 2026 in Portuguese, English and Spanish were included, resulting in a final sample of 10 articles. Results and Discussion: The findings revealed that frequent exposure to death, patient and family suffering, work overload, institutional weaknesses and insufficient professional training are the main factors associated with occupational stress. Consequences included anxiety, emotional exhaustion, moral distress, professional burnout and impairment in care quality. Coping strategies identified included self-care, institutional support, continuing education, psychological support, spirituality and strengthening interpersonal relationships. Conclusion: Occupational stress in palliative care arises from emotional, organizational and care-related factors, highlighting the need for institutional strategies focused on mental health promotion, professional qualification and improved working conditions for nurses.

Resumen

Introducción: El estrés ocupacional en enfermeros que actúan en cuidados paliativos constituye un importante problema de salud laboral, asociado a la exposición continua al sufrimiento, la terminalidad y las elevadas demandas emocionales propias de este contexto asistencial. Objetivo: Analizar los factores asociados al estrés ocupacional en enfermeros que trabajan en cuidados paliativos, considerando sus repercusiones en la práctica asistencial y en la salud psicofísica de estos profesionales. Metodología: Se trata de una revisión integradora de la literatura realizada en las bases SciELO, LILACS, PubMed y Google Scholar. Se utilizaron los descriptores “estrés ocupacional”, “cuidados paliativos”, “salud del trabajador”, “enfermero” y “sufrimiento laboral”, combinados mediante los operadores booleanos AND y OR. Fueron incluidos estudios publicados entre 2021 y 2026 en portugués, inglés y español, totalizando 10 artículos seleccionados. Resultados y Discusión: Los estudios demostraron que la convivencia frecuente con la muerte, el sufrimiento de pacientes y familiares, la sobrecarga laboral, las fragilidades institucionales y la insuficiente capacitación profesional constituyen los principales desencadenantes del estrés ocupacional. También se identificaron repercusiones como ansiedad, agotamiento emocional, sufrimiento moral y disminución de la calidad asistencial. Entre las estrategias de afrontamiento destacaron el autocuidado, el apoyo institucional, la educación continua, el apoyo psicológico, la espiritualidad y las relaciones interpersonales saludables. Conclusión: El estrés ocupacional en cuidados paliativos está influenciado por factores emocionales, organizacionales y asistenciales, siendo necesarias acciones dirigidas a la promoción de la salud mental y a la mejora de las condiciones laborales de los enfermeros.
