TRANSITION OF CARE IN MENTAL HEALTH AND THE ROLE OF THE FAMILY AFTER DISCHARGE

Object: The transition of care for patients with mental disorders after hospital discharge represents a critical moment in the therapeutic process, often marked by a lack of guidance for family members, which increases the risk of treatment discontinuity and relapse. Objective: This study aims to identify, through an integrative literature review, the main scientific evidence on the transition of mental health care after hospitalization, with a focus on the family’s role. Methodology: Integrative literature review, following the six steps of Evidence-Based Practice (EBP), using the PICO strategy for the guiding question. An initial set of 56 articles was reviewed, from which 13 were selected according to the inclusion criteria. Theoretical Approach: Evidence-Based Practice (EBP). Results: Highlight the need for educational interventions that provide information on treatment adherence, signs of relapse, access to the Psychosocial Care Network (RAPS), active listening, stigma reduction, and practical guidelines for daily care. Conclusion: The education booklet for family members, using accessible language, technical-scientific content, and a humanized approach, is an essential tool bond and reduce vulnerabilities and the risk of readmissions in the post-discharge period.
