This article aimed to analyze the morbidity and mortality from malignant brain neoplasms in the Southeast Region of Brazil compared to the national panorama, considering the period from 2015 to 2024. The research consisted of a time-series study conducted with secondary data from the Department of Informatics of the Unified Health System (DATASUS). The analysis evaluated demographic variables, therapeutic modalities, and clinical outcomes. The results showed that the Southeast Region concentrated the largest proportion of cases in the country, corresponding to more than forty percent of national records. A predominance was observed in males and in the 50 to 64 age group, with radiotherapy being the most frequent initial therapeutic modality. Although the Southeast showed greater agility in starting oncological treatment, the regional hospital mortality rate exceeded the national average. A significant racial disparity was also found, with higher mortality rates in the Black population. The conclusion indicates that, despite the advanced infrastructure, there are profound inequalities in the prognosis of the disease. Therefore, it is imperative to improve epidemiological surveillance strategies and formulate public policies focused on equity and access to oncological treatments.
