Brazil is undergoing a demographic and epidemiological transition characterized by population aging and an increase in Non-Communicable Chronic Diseases (NCDs), a reality that is even more worrying in more vulnerable regions, such as the Sertão Paraibano. In this context, Food and Nutrition Education (FNE) and nutritional assessment are fundamental strategies in Primary Health Care (PHC), promoting autonomy, preventing health problems, and improving quality of life. This study aimed to analyze the nutritional profile of the adult and elderly population served by a Family Health Unit, as well as to evaluate the contributions of FNE actions carried out by the Sertão Cuidado Cardio Project. The descriptive and exploratory research identified a profile marked by socioeconomic vulnerability, a high prevalence of overweight, hypertension, and diabetes. The educational actions, carried out through participatory methodologies such as discussion groups and playful materials, proved effective in promoting self-care and the adoption of healthier eating habits. It is concluded that the Sertão Cuidado Project represents a successful experience of interdisciplinary and humanized care, strengthening the role of Primary Health Care in the prevention of NCDs and in the promotion of health in the Sertão region.
