Given the expanding use of tirzepatide in obesity management, it is understood that changes in satiety directly impact dietary adherence and therapeutic success, making specialized support essential to avoid nutritional deficiencies and weight regain. This study analyzes the impacts of changes in satiety and appetite resulting from tirzepatide on dietary adherence, as well as the role of nutritional counseling in modulating these effects. Specifically, it seeks to identify the neuroendocrine effects of the drug, analyze its influences on eating behavior, describe adherence difficulties, and investigate appropriate dietotherapy strategies. Methodologically, this is a qualitative narrative literature review based on searches in electronic scientific databases between 2018 and 2026, organizing data into thematic axes on physiology, side effects, and nutritional management. Results show that tirzepatide reduces appetite and food noise, but causes nausea and early satiety, increasing the risks of covert malnutrition and secondary sarcopenia. Because strictly pharmacological and passive weight loss does not ensure long-term maintenance of results, individualized nutritional counseling is established as an essential moderating element to mitigate gastrointestinal discomfort, adjust protein intake, and ensure intervention sustainability. It is concluded that tirzepatide is effective in weight management; however, appetite suppression and digestive changes require structured dietary intervention. Individualized nutritional monitoring proves essential to mitigate side effects, preserve lean mass, consolidate new healthy habits, and prevent weight regain.
