Fentanyl is a highly potent synthetic opioid used primarily in hospital settings for analgesia and anesthesia. However, its irrational use has become a serious public health problem, associated with increased rates of addiction, overdose, and deaths. This study aims to analyze the scientific literature on the irrational use of fentanyl from 2018 to 2025, focusing on its epidemiological, social, and health impacts, both internationally and nationally. The methodology such as PubMed, SciELO, and LILACS. The results indicate that fentanyl was responsible for a significant increase in opioid-related deaths in the United States, being identified as the main drug involved in overdoses since 2018. In Brazil, although non-medical use is still emerging, seizures, adulterations of illicit drugs, and reports of recreational use have already been recorded. Countermeasures include epidemiological surveillance, professional training, clinical prevention protocols, and the provision of naloxone as an antidote. The conclusion is that, although the irrational use of fentanyl is more advanced in the US, Brazil is already showing worrying signs that require immediate responses.
