Abstract

Psoriasis is a chronic inflammatory skin disease that affects approximately 2% of the world's population. Obesity is a multifactorial chronic disease that has increased significantly worldwide, and has been associated with worsening psoriasis severity. This article aims to review the available evidence on the relationship between psoriasis and obesity, and provides recommendations for the appropriate management of these conditions together. a systematic review of the literature, which encompasses research and analysis of scientific articles and systematic reviews published in electronic databases such as PubMed, Scopus and Web of Science. The search strategy will include the terms "psoriasis", "obesity", "treatment", "management", "recommendations", "weight", "body mass index", "lifestyle" and "interventions". The search will be limited to articles published in English and Portuguese, and studies carried out in human beings. The prevalence of obesity in patients with psoriasis is higher than in the general population. Obesity is also associated with worsening the severity of psoriasis, including greater extent of the lesion, increased risk of psoriatic arthritis, and poorer response to treatment. Weight loss can improve psoriasis symptoms and reduce systemic inflammation. Different approaches to weight loss can be effective in patients with psoriasis, including a low-fat, low-carb diet, the Mediterranean diet, intermittent fasting, and bariatric surgery. In addition, physical activity can improve psoriasis and obesity by reducing oxidative stress and inflammation. Proper management of psoriasis and obesity should include a multidisciplinary approach, involving a dermatologist, a nutritionist and a physical educator. It is important to consider the presence of comorbidities in patients with psoriasis and obesity, such as cardiovascular disease and diabetes, and adjust therapy accordingly. In addition, promoting a healthy lifestyle can have benefits for the skin and overall health. The relationship between psoriasis and obesity is complex and requires an integrated approach for proper management. Weight loss and physical activity can improve the severity of psoriasis and reduce the risk of obesity-related comorbidities. We recommend adopting a healthy lifestyle as part of psoriasis treatment in overweight or obese patients.
