In endodontics, irrigation of the root canals is one of the most important steps, since the debridement and disinfection of the root canal system is performed. The extrusion of sodium hypochlorite into the periradicular tissues can be one of the most alarming accidents, because of its immediate clinical manifestations, causing intense pain and instant edema. The aim of the present study is to review the literature on accidents caused by the irrigation process of root canals with sodium hypochlorite during endodontic treatment. The work is composed of a literature review and an exploratory research. We conclude with the present study that sodium hypochlorite solutions, when inadvertently injected into the periapical region, cause tissue damage, discomfort for the patient and, consequently, doubts about the skill of the dentist. In addition, the professional must know how to identify the problem and take quick measures in order to cause less harm to the patient.
