This article aimed to analyze the prevalence and etiological factors associated with failures in endodontic treatments, with emphasis on the influence of clinical, technical, and biological factors, between 2010 and 2024. The study was conducted through a bibliographic and documentary review of an exploratory nature, based on the analysis of scientific studies published in national and international databases. The results showed that therapeutic failures are mainly related to the persistence of microorganisms in the root canal system, especially resistant biofilms, as well as technical inadequacies during the stages of instrumentation, disinfection, and obturation, and the anatomical complexity of the canals. It was also observed that, although there has been a reduction in the prevalence of failures over the years, these events remain associated with multifactorial causes. It is concluded that the understanding of these factors, combined with the proper execution of techniques and continuous professional updating, is essential for improving clinical outcomes in endodontics.
