Introduction: generalized pustular psoriasis is a rare, severe, and potentially fatal form of psoriasis characterized by flares of widespread sterile pustules, systemic inflammation, and risk of organ complications. Objective: to identify and discuss the main complications associated with generalized pustular psoriasis in literature published between 2021 and 2026. Methodology: rapid qualitative systematic review conducted according to PRISMA 2020, with searches in PubMed, BVS, and SciELO using DeCS/BVS descriptors related to pustular psoriasis, complications, comorbidity, hospitalization, and mortality. Results: five studies were selected. The most recurrent complications included hospitalization for acute flares, sepsis or suspected infection, electrolyte disturbances, hypoalbuminemia, hypocalcemia, hepatic, renal, respiratory, and cardiovascular dysfunction, as well as mental health, quality of life, and mortality impact. Conclusion: generalized pustular psoriasis should be understood as a severe systemic inflammatory disease, not merely an extensive skin disorder. Early recognition of complications is essential to reduce hospitalization, organ failure, and mortality.
