ASSOCIATION BETWEEN AORTIC COARCTATION AND INFANTILE AIRWAY HEMANGIOMA: A CASE REPORT

Objective: To report a case of association between aortic coarctation and infantile hemangioma with airway involvement. Methodology: Descriptive clinical case report. Case Report: A female patient presented with early episodes of dyspnea and perioral cyanosis. During hospitalization for respiratory symptoms associated with SARS-CoV-2 infection, a heart murmur was identified, prompting further investigation that confirmed aortic coarctation. Concomitantly, videonasolaryngoscopy revealed a hemangioma in the oropharyngeal and subglottic region, responsible for stridor and partial airway obstruction. The patient underwent surgical correction of the coarctation by aortoplasty with placement of a diamond patch, with good postoperative evolution. The hemangioma was treated with propranolol, showing significant regression of the deep lesions. In late outpatient follow-up, serial echocardiographic examinations showed significant narrowing of the aortic isthmus compatible with recoarctation. Conclusion: The association between aortic coarctation and hemangioma with airway compromise can result in a clinical presentation initially dominated by respiratory symptoms, hindering the early recognition of congenital heart disease. This case reinforces the importance of comprehensive clinical evaluation in infants with persistent respiratory symptoms and cardiological follow-up after coarctation repair. The efficacy of propranolol in the management of hemangiomas with functional impact and the relevance of a multidisciplinary approach to care are also highlighted.
