This article critically analyzes the discrepancy between the National Tactical Pre-Hospital Care Guideline, established by Ordinance No. 98/2022, and the operational reality consolidated in the Military Police of Paraná (PMPR). The research adopts a qualitative approach, with normative, doctrinal, and institutional analysis, examining the basic training of enlisted personnel, standard operating procedures, and PMPR Guideline No. 014/2025. It was found that, although the federal regulation represents progress in standardizing care in hostile environments, it proves insufficient given the level of technical maturity already achieved by state institutions. The PMPR presents a model structured in three levels—training, standardization, and execution—evidencing that the main problem of Tactical Pre-Hospital Care in Brazil does not lie in the absence of technical knowledge, but in the lack of normative consolidation at the legal level. It concludes that there is a need for specific federal legislation that ensures legal security for agents and effectiveness in protecting life.
