This is an epidemiological, descriptive, and cross-sectional study aimed at analyzing the mortality profile of children under one year of age in the health regions of the state of Paraná, Brazil, from 2015 to 2024. Data were obtained from the Mortality Information System (SIM) and the Live Birth Information System (SINASC), with the infant mortality rate per thousand live births being calculated. Analyses were stratified according to sex, race/color, age and maternal education, age components of death, and causes according to the International Classification of Diseases (ICD-10). During the period, 15,280 infant deaths were recorded, with a predominance of males. Heterogeneity was observed among the health regions, with higher rates in regions such as Umuarama, Ivaiporã, and Cornélio Procópio, and lower rates in the Metropolitan Region and Cascavel. The early neonatal component stood out as the main cause of death. A higher risk was observed among children of mothers at the extremes of reproductive age, with low levels of education, and belonging to the indigenous population. The main causes were perinatal conditions and congenital malformations, highlighting social inequalities.
