RECONSTRUCTIVE APPROACH TO LOWER EYELID ECTROPION WITH AURICULAR GRAFT: ANALYSIS OF TECHNIQUES AND RESULTS

Lower eyelid ectropion is a condition characterized by eversion of the eyelid margin, resulting in ocular exposure, epiphora, and functional and aesthetic impairment. Its etiology is multifactorial, commonly associated with involutional, cicatricial, paralytic, or iatrogenic causes. Among the available therapeutic approaches, full-thickness skin grafts stand out as one of the main options for the correction of cicatricial ectropion, particularly in cases involving tissue loss or significant skin contraction. The auricular region, especially the postauricular area, is widely used as a donor site due to its similarity to eyelid skin, in addition to its low morbidity and favorable aesthetic outcomes. This study aims to analyze, through a literature review, the role of auricular grafts in lower eyelid reconstruction for ectropion correction, addressing surgical techniques, indications, complications, and clinical outcomes. Evidence suggests that the combination of skin grafts with cartilage grafts and eyelid support techniques provides greater structural stability and improved functional results. However, complications such as graft necrosis, scar retraction, and recurrence remain significant challenges. In conclusion, auricular grafting represents an effective, safe, and widely used option in clinical practice, particularly when applied within individualized and combined reconstructive approaches. Treatment success depends on proper surgical planning, anatomical knowledge, and appropriate management of potential complications.
