Objective: To analyze the role of health indicators as management tools in Primary Health Care (PHC) in the management of Non-Communicable Chronic Diseases (NCDs), focusing on multiprofessional articulation, health education, and user autonomy. Methods: An integrative literature review was conducted in the PubMed, SciELO, and LILACS databases, covering the period from 2020 to 2026. The selection included original articles, guidelines from medical societies, and documents from the Ministry of Health. Results: Performance indicators, such as those of the Previne Brasil Program, optimize the early identification of health problems and the organization of the care flow. Interprofessional practice and the use of digital technologies enhance metabolic control and reduce hospitalizations. Health education, through active methodologies, proved fundamental to increasing health literacy and therapeutic adherence. Discussion: A tension is observed between productivist management and humanized clinical practice. User autonomy is central to the sustainability of care, requiring that quantitative indicators be integrated with the qualitative perception of well-being. Conclusion: The convergence between qualified management, cohesive multidisciplinary teams, and user empowerment is essential for the resilience of the Brazilian Unified Health System (SUS) in the face of the epidemiological burden of NCDs.
