This article aimed to investigate the relevance of practicing the Pilates method during pregnancy, exploring its benefits, contraindications, and impact on maternal-fetal health, with the goal of systematizing knowledge for professionals and pregnant women. A qualitative narrative literature review was conducted, with a systematic search in the SciELO, LILACS, and Google Scholar databases, using the descriptors "Pilates Method," "Physical Exercise," and "Pregnant Woman." Original articles, reviews, books, dissertations, and theses published in Portuguese between 2015 and 2025 were included. The results indicate that the Pilates method, when adapted and supervised, significantly improves maternal and fetal health, mitigating musculoskeletal pain (low back and pelvic pain) by strengthening the deep trunk muscles and improving posture. It also contributes to strengthening the pelvic floor, optimizes cardiovascular and respiratory functions, and promotes mental well-being, reducing stress and anxiety. In conclusion, the Pilates method is an effective multifaceted intervention, but it requires individualization and professional supervision to ensure safety and avoid risks, highlighting the need for more controlled studies.
