Introduction: The transition of palliative care for children with an intestinal elimination ostomy is a complex process that involves changes in the care plan, family dynamics, and nursing practice. In this context, nurses play a central role in ostomy management, communication with children and their families, and psychosocial support, facing challenges that directly impact the continuity and quality of care. Objective: To discuss the challenges of nursing in the transition of palliative care for children with an intestinal elimination ostomy. Methodology: This is a descriptive bibliographic review with a qualitative approach, based on the analysis of scientific publications related to pediatric palliative care, intestinal ostomies, and nursing practice. Analysis and discussion of results: The findings allowed the construction of thematic categories highlighting communication difficulties during the care transition process, care demands related to ostomy management, psychosocial impacts experienced by children and their families, and the need for support strategies for nursing professionals in the face of emotional and technical overload. Conclusion: The challenges faced by nursing in the transition of palliative care for children with an intestinal ostomy require a comprehensive and humanized approach, including effective communication, family support, professional qualification for specialized care, and the strengthening of institutional strategies to support nurses, ensuring continuity, safety, and quality of pediatric palliative care.
