The pursuit of dental aesthetics has increased significantly, driven by social standards that value aesthetics in general. In this context, tooth whitening stands out as one of the most frequently performed procedures in modern dentistry, due to its good results and safety when correctly applied. However, tooth sensitivity is the main adverse effect, potentially compromising patient comfort. This study analyzed the impact of tooth whitening on tooth sensitivity, considering the biological mechanisms involved, the most commonly used substances, such as hydrogen peroxide and carbamide peroxide, their concentrations, clinical indications, and methods for controlling sensitivity. This is a narrative literature review based on articles published between 2012 and 2022. The results indicate that, although whitening is effective and safe, sensitivity is a transient effect associated with the diffusion of whitening agents into the dentinal tubules. It is concluded that mastering biological mechanisms, choosing the appropriate technique, and using desensitizing agents are fundamental to reducing adverse effects and ensuring satisfactory aesthetic and functional results.
