Pharmacists perform various functions related to the proper management of medications, aiming to improve quality of life through pharmacotherapy. Therefore, they are qualified professionals to develop appropriate treatment. Effective communication among healthcare team members is essential for the success of pharmaceutical interventions and therapeutic monitoring. Objectives: The general objective of this study is to address the impact of pharmacists' work on preventing hospital readmissions and providing medication guidance upon discharge. The specific objectives are: To identify the activities performed by pharmacists in hospital settings; To describe medication reconciliation; To mention the role of pharmacists in prescription review; To highlight the importance of pharmaceutical counseling upon hospital discharge. Methodology: A bibliographic review of the literature covering the years 2020 to 2025 was used as the methodology, following ABNT formatting standards. Conclusion: Pharmaceutical assistance at the time of hospital discharge is essential to ensure continuity of treatment at home, reducing the possibility of medication errors and ensuring the appropriate use of medications.
