Congenital malformations of the circulatory system are structural anomalies of the heart and blood vessels that occur during fetal development. These conditions represent a significant cause of infant mortality and can result in serious health problems later in life. In Brazil, monitoring these malformations is crucial for developing public health policies and preventive interventions. The state of Paraná has a robust birth and malformation surveillance system, allowing for detailed analysis of the incidence of these conditions. This study aimed to analyze the incidence of congenital malformations of the circulatory system in Paraná from 2020 to 2023. A total of 523 cases were recorded in a population of 569,538 live births, with a slight prevalence in females. The cities with the highest number of cases were Londrina, Curitiba, and Maringá. A higher concentration of cases was observed in the maternal age group of 25 to 34 years. The incidence increased from 8.33 to 11.21 per 10,000 live births between 2020 and 2023.
