Hydrops of the fetal adnexa is a pathological condition characterized by excessive accumulation of fluid in the uterus, which may occur in the allantois or amnion, and mainly affects cattle. This condition can result in serious complications during pregnancy, such as dystocic births, compromising the health of the cow and the fetus. The objective of this research was to report a case of hydrops of the fetal adnexa in a Holstein cow, highlighting the diagnosis, treatment and outcome of the case. The methodology involved performing anamnesis, clinical examinations, rectal palpation and ultrasound, which confirmed the diagnosis of hydroallantois. The treatment consisted of the controlled drainage of approximately 80 liters of allantoic fluid and subsequent performance of a cesarean section to remove the fetus. The calf was born alive, however, as the gestation was interrupted due to the health of the cow, which was compromised and because it was premature, it died after a few hours. The cow underwent appropriate sutures and received postoperative treatment with anti-inflammatories and antibiotics, without compromising its general condition. The study concluded that early diagnosis, associated with appropriate therapeutic interventions, such as drainage and cesarean section, was effective in ensuring the survival of the animal and the fetus, demonstrating the relevance of these procedures in cases of hydrops of fetal adnexa.
