Cysts of neoplastic origin in ovaries can coexist in their malignant or benign form, both of which cause signs of compression in neighboring organs, such as the bladder or colon, in their giant forms. This report describes the case of a female patient, 39 years old, G2P2A0, presenting with progressive pain in the hypogastric region, without signs of intestinal changes. He presented a massive cystic expansion on computed tomography of the pelvis, without changes in blood count, CA 125, CA 19-9 and CEA. Subsequently, exploratory laparotomy was indicated, revealing a large bilateral ovarian cyst, and bilateral adnexectomy and total abdominal hysterectomy were chosen. Finally, given the histopathological diagnosis of “like” endocervical mucinous cystadenocarcinoma in the left ovary and mucinous cystadenoma in the right ovary. The patient presented good clinical evolution, being discharged from hospital and continuing with outpatient follow-up by the oncology service. The present report is important to elucidate the clinical presentation of a rare pathology and insidious symptoms. Therefore, due to its asymptomatic forms, it is necessary to have its diagnosis and management disseminated for the broad knowledge of the first care clinical team, mainly gynecologists and oncologists.
