HOME CARE: CHALLENGES AND ROLE OF THE NURSING TEAM IN PROVIDING PERSONALIZED CARE TO PATIENTS

Abstract

This literature review article aims to discuss nursing care in home care, highlighting the obstacles faced by these patients and the functions of each professional in the nursing team. Through the analysis of scientific articles, the main difficulties faced by patients were identified, such as social isolation, lack of family support, and mobility problems. The role of each nursing professional in this context was emphasized, including the nurse, nursing technician, and nursing assistant. The main difficulties faced by professionals in home care were identified, such as lack of material and human resources, and lack of security, among others. Possible solutions to these difficulties were discussed, such as improving communication among team members, using assistive technologies, and adopting preventive measures.
