Endodontic revascularization aims to promote tissue repair in teeth with pulp necrosis. In this context, platelet-rich fibrin (PRF) has been investigated as an autologous scaffold because of its three-dimensional structure and release of bioactive mediators. This study aimed to analyze the biological basis, clinical applications, outcomes, advantages, and limitations of PRF in endodontic revascularization. A qualitative narrative literature review was conducted using PubMed/MEDLINE, SciELO, and LILACS, covering publications from 2021 to 2026. Seventeen publications were included, of which 13 directly informed the results and discussion. The studies reported applications in mature and immature permanent teeth, with promising outcomes regarding periapical healing and, in immature teeth, continued root development. However, consistent superiority of PRF over blood clots was not demonstrated, and clinical and radiographic improvement does not establish true pulp regeneration. Heterogeneity in clinical protocols limits comparisons across studies. PRF is therefore a biologically plausible and promising alternative, although more standardized research with long-term follow-up and functional and histological assessments is needed.
