Introduction: The manipulation of antineoplastic drugs requires strict biosafety and quality standards due to their high toxicity. The pharmacist plays a key role in ensuring patient safety, therapeutic effectiveness, and humanized cancer care. General objective: To analyze pharmaceutical management in the manipulation of antineoplastics, highlighting regulatory standards and quality strategies that ensure safe and effective processes. Methodology: This study was conducted through an integrative literature review with a qualitative and descriptive approach. Searches were carried out in SciELO, PubMed, and Google Scholar, considering publications from 2023 to 2025, using descriptors in Portuguese and English related to pharmaceutical management and antineoplastics. Results: Findings showed that the pharmacist’s presence reduces medication errors, strengthens traceability, and improves treatment adherence. Strategies such as compliance with ANVISA regulations, implementation of quality management systems (ISO 9001, PBQP), continuous training, and automation contribute to safer practices for professionals and patients. In addition, humanized care combined with technological innovation enhances trust and treatment effectiveness. Conclusion: Pharmaceutical management in the manipulation of antineoplastics is a strategic axis in contemporary oncology. The pharmacist’s role integrates science, technology, and humanized care, ensuring standardized processes, risk reduction, and strengthening of the relationship between patient, team, and institution.
