The Objective of research was emphasize the existence and rarity of bladder pseudoextrophy and correlate with the importance of physical examination for diagnosis. Methods: Report a case through the Study of patient records, exams and periodic appointments. Results and discussion: Bladder pseudoextrophy, rare variant of bladder extrophy, consists in a structuraly normal bladder, covered by a epithelial membrane, that herniates through abdominal wall during efforts. The many patient complaints, added to the physical exam signs, led to the investigation of the urinary tract and detection of: hiperactive bladder with diverticulum and voiding residue recurrent epididymo-orchitis and varicocele. Conclusion: The value of a well executed physical exam led us to notice a silent incidious disease, that has big potential to decrease renal function and lower fertility. Follow up is fundamental for the right development of this patient and for preventing complications through out his life.
