TELEPHONE-BASED CARE IN PRIMARY HEALTH CARE FOR PEOPLE WITH NON-COMMUNICABLE DISEASES: AN EXPERIENCE REPORT

Abstract

Objective: This study aims to elucidate the telehealth service experience in addressing demands pertaining to chronic diseases in the adult and elderly populations within the healthcare network of the municipality of São Leopoldo/RS. Method: A comprehensive experiential report is presented, detailing the implementation of telehealth services for patients afflicted with chronic diseases, grounded in an interdisciplinary health framework. Results: The reported experience shed light on the telehealth program, specifically the telehealth services for users with chronic disease-related demands in the municipality under study, originating during the Covid-19 pandemic.  Discussion: Telehealth consultations have emerged as a pivotal technological resource for the management, rehabilitation, continuous monitoring, education, and health promotion among individuals diagnosed with chronic diseases, particularly in cases of chronic arterial hypertension and diabetes mellitus, among others. Conclusion: The implementation of digital care has facilitated access to healthcare, overcoming barriers such as mobility and accessibility. Telehealth favors multidisciplinary work, ensuring comprehensive care and efficient communication between professionals. To enhance the project, it is essential to invest in public policies, training, infrastructure and research, promoting the integration between technology, education and health practices.
