The incorporation of Artificial Intelligence (AI) into the Brazilian Unified Health System (SUS) has become a strategic element for the digital transformation of Brazilian public health. This study aims to analyze, through a systematic literature review, the contributions, challenges, and perspectives of AI application in the SUS. The search was conducted in the CAPES Portal, SciELO, and Google Scholar databases, using the descriptors (“AI” and SUS) OR (“Unified Health System” AND technological innovation). Studies addressing AI applied to management, healthcare, and decision-making were selected, considering technical, ethical, and institutional aspects. The results show that AI has high potential to optimize processes, improve diagnostic accuracy, support resource management, and expand access to health services. However, challenges related to technological infrastructure, professional qualification, data governance, and regulation persist. It can be concluded that the adoption of AI in the Brazilian Unified Health System (SUS) can strengthen the system's efficiency, equity, and sustainability, provided it is accompanied by strategic planning, appropriate public policies, and ethical commitment.
