Introduction: Brain death is defined as the irreversible cessation of brain functions, being legally recognized as death in Brazil and constituting a fundamental milestone in the process of organ and tissue donation. In this context, the hemodynamic maintenance of the potential donor represents one of the main challenges faced by the nursing team, since physiological alterations resulting from brain death may compromise the viability of organs for transplantation. Objective: To analyze the challenges faced by nurses in the hemodynamic maintenance of potential organ donors with brain death, as well as their role in family support and in the donation process.Methodology: This is a descriptive literature review with a qualitative approach, carried out through the analysis of scientific publications published between 2011 and 2025. Analysis and discussion of results: The results show that hemodynamic instability, associated with complications such as hypotension, metabolic disorders, and diabetes insipidus, requires continuous monitoring and rapid nursing interventions. Furthermore, the importance of humanized communication with family members is highlighted, as it is an essential factor for understanding the diagnosis and obtaining authorization for organ donation. Conclusion: It is concluded that the nurse's role is fundamental both in preserving organ quality and in providing family support, being indispensable for maintaining the viability of organs intended for transplantation and strengthening the organ donation process.
