Diabetes mellitus is an important public health problem due to its high epidemiological magnitude and association with metabolic and cardiovascular complications. This study aimed to analyze mortality from endocrine, metabolic and circulatory system diseases associated with diabetes mellitus in the Eastern Health Macro-region of Paraná, from 2019 to 2023. This is an ecological, retrospective, descriptive study with a quantitative approach, based on secondary data from the Mortality Information System (SIM/DATASUS). A total of 26,526 deaths were analyzed in the municipalities of Curitiba, Guarapuava, Paranaguá and Ponta Grossa. A higher concentration was observed in Curitiba, an increasing trend until 2022 and a reduction in 2023. There was a predominance in the elderly, a slightly higher frequency in males and a concentration among individuals with lower education. Circulatory system diseases stood out as the main causes associated with diabetes, highlighting the need for prevention, control and health surveillance actions.
