Abstract

Introduction: This study aimed to analyze the contribution of the clinical pharmacist to therapeutic adherence in hypertensive elderly patients aged 70 to 80 years. General Objective: The research, qualitative in nature and descriptive in character, was based on an integrative literature review in databases such as SciELO, LILACS, PubMed, and Google Scholar, covering publications between 2019 and 2025. Methodology: The study was developed from the analysis of recent scientific evidence, allowing the identification of risk factors and intervention strategies. Discussion and Results: The findings showed that adherence to treatment is strongly compromised by factors such as polypharmacy, cognitive difficulties, socioeconomic limitations, and low educational level, which favor therapeutic discontinuity and increase the risk of cardiovascular complications. It was observed that clinical pharmaceutical follow-up promotes effective interventions, including prescription review, pharmacotherapeutic monitoring, health education, and accessible communication strategies, which resulted in greater adherence, reduction of hospital admissions, and better quality of life for the elderly. In addition, the integration of the pharmacist into multiprofessional teams, through standardized clinical protocols, shared electronic medical records, and continuing education programs, demonstrated potential to optimize health outcomes and reduce costs for the public system. Conclusion: It is concluded that strengthening the role of the clinical pharmacist is essential to overcome barriers to therapeutic adherence, ensure the rational use of medicines, and promote humanized and efficient care in the management of hypertension in elderly patients.
