Blood transfusion is vital for preserving public health, being crucial in surgical procedures, emergencies, and the treatment of chronic diseases. However, the scarcity of voluntary blood donors in Brazil compromises the supply of blood products and other components, especially during critical periods. This study aimed to analyze, through statistical data, the causes of clinical ineligibility among blood donors in the hemotherapy service of Piauí, between January and December 2024. 2,224 blood donor screening forms were analyzed, considering variables such as sex, age, and types of ineligibility. The results showed that the main causes were low hemoglobin, risky behavior, and endoscopy. This is an exploratory documentary research, considering data referring to the period from January to December 2024. Donor selection, based on clinical, epidemiological, and laboratory criteria, aims to ensure the protection of both the donor and the recipient. Analysis is crucial for suggesting more effective awareness and recruitment strategies, considering the socioeconomic and cultural particularities of the state of Piauí.
