Introduction: Cardiovascular diseases (CVD) are among the leading causes of morbidity and mortality in the elderly, posing an increasing challenge to health systems due to population aging. In this context, pharmacists play a crucial role in prevention and management through educational activities and guidance on the rational use of medicines. General Objective: To analyze the contribution of pharmacists in the prevention and control of cardiovascular diseases in the elderly, with emphasis on health education. Methodology: This study was carried out through an integrative literature review, with a qualitative and descriptive approach. Databases such as SciELO, PubMed, LILACS, Google Scholar, and VHL were consulted, including articles published between 2023 and 2025, in Portuguese and English, addressing pharmacists’ role in elderly CVD care. Results: Evidence shows that health education conducted by pharmacists enhances therapeutic adherence, reduces blood pressure, improves lipid and glycemic profiles, and decreases hospital admissions. Best practices identified include individualized counseling, educational groups, pharmacotherapeutic follow-up, and the use of digital technologies. Conclusion: Pharmacists’ role in health education is essential to reduce cardiovascular morbidity and mortality in the elderly, promote self-care, and improve quality of life. Investments in public policies, continuous training, and innovative technologies are necessary to overcome structural barriers and expand the impact of these practices.
