This article sought to investigate the factors associated with resistance to syphilis treatment and explore new therapeutic alternatives that may contribute to improving infection control. In Brazil, cases of acquired syphilis increased by 742.47% between 2011 and 2021, highlighting the need for effective diagnostic, treatment, and prevention strategies. Given this scenario, this research consists of an integrative literature review, with a qualitative and descriptive approach, aiming to analyze the factors associated with resistance to syphilis treatment and explore recent therapeutic alternatives. The search for materials was conducted in recognized scientific databases, considering publications between 2015 and 2025. Original articles, qualitative and quantitative studies, narrative reviews, and clinical guidelines were included, while non-indexed publications and non-peer-reviewed studies were excluded. Data analysis was organized into thematic axes, allowing for an in-depth understanding of the challenges in syphilis management and the new therapeutic approaches under development. The research contributed to updating knowledge on the topic and supported strategies to improve clinical care and the formulation of effective public policies to combat antimicrobial resistance.
