This article analyzed the importance of pharmacotherapeutic monitoring for systemic arterial hypertension in the elderly, emphasizing its contribution to clinical control of the disease and promoting quality of life for this population. The methodology consisted of an integrative literature review conducted between 2014 and 2025 in the SciELO, Virtual Health Library (VHL), PubMed, and LILACS databases. Descriptors in Portuguese and English were used with Boolean operators "AND" and "OR." After applying the inclusion and exclusion criteria, 10 articles were selected for the final analysis. The results indicated that pharmaceutical intervention is effective in improving treatment adherence, identifying potential drug interactions, and reducing adverse events. It was also observed that monitoring conducted in community pharmacies, nursing homes, and primary care services showed a positive impact on therapeutic safety and blood pressure control. It is concluded that pharmacotherapeutic monitoring is an essential strategy in multidisciplinary care, with the potential to reduce complications associated with hypertension in the elderly, optimize the rational use of medications, and contribute to healthier aging.
