The postpartum period represents a phase of intense physical, emotional, and social transformations in a woman's life, directly impacting her mental health and sexuality. Disorders such as postpartum depression, anxiety, and sexual dysfunction are common in this context and are often overlooked in healthcare. A comprehensive understanding of these aspects is essential to promote more humanized and effective care in the postpartum period. In this context, this study aimed to systematically review the scientific literature on the relationship between mental health and sexuality in postpartum women, identifying risk factors, prevalence, and care strategies. This is a systematic review based on articles published between 2021 and 2025, selected from the PubMed, BVS, and LILACS databases, using the descriptors "postpartum period," "mental health," "sexuality," and "women." After applying the eligibility criteria and analyzing the full texts, 16 studies were included. The results indicated a high prevalence of depressive and anxiety symptoms, with a negative impact on sexual desire, arousal, and marital satisfaction. Associated factors included a prior history of mental disorders, difficulties bonding with the baby, hormonal changes, and a lack of psychosocial support. The lack of integrated approaches in health services and the invisibility of sexual complaints in the postpartum period were identified as significant barriers. The conclusion is that postpartum care should incorporate systematic assessment of mental health and sexuality, with interdisciplinary actions that are sensitive to individual needs. Educational strategies, qualified support, and strengthening primary care are essential to ensure comprehensive and equitable care.
