This article investigates the epidemiological profile of gestational and congenital syphilis in the 10th Regional Health District of Cascavel, from 2018 to 2023, comparing it with state and national averages. Syphilis, though preventable, continues to affect pregnant women, resulting in congenital syphilis and health risks for newborns. Data were collected from SINAN and the results indicated a high incidence of gestational syphilis in the 10th Regional Health District of Cascavel, possibly due to improvements in surveillance and expanded diagnostics. In Brazil, the incidence of congenital syphilis was higher, suggesting shortcomings in the prevention of vertical transmission. However, Paraná and 10th Regional Health District of Cascavel showed lower rates of congenital syphilis, pointing to more effective prenatal practices. It is concluded that quality prenatal care, along with partner notification and treatment, are essential in preventing congenital syphilis. The disparity between regions reinforces the need for an equitable healthcare system with targeted interventions. Strengthening Primary Care and Family Health is essential to reduce infant morbidity and mortality associated with congenital syphilis.
