This article aims to analyze the scientific literature regarding the role of ultrasound in diagnosing major uterine and ovarian anomalies, focusing on conditions such as fibroids, endometriosis, and other gynecological disorders. It is a narrative literature review based on publications from PubMed, SciELO, official documents, and journals provided by the Brazilian Federation of Gynecology and Obstetrics Associations (FEBRASGO), as well as clinical protocols and therapeutic guidelines from the Ministry of Health, covering the period from 2010 to 2025. This review seeks to assess the effectiveness of different ultrasound modalities — including transvaginal, Doppler, and three-dimensional ultrasound — highlighting their clinical applications, benefits, and limitations in gynecological practice. It concludes that ultrasound, due to its accessibility and safety, remains essential for the screening, diagnosis, and follow-up of gynecological conditions, supporting more assertive clinical decision-making.
