Abstract

Complex incisional hernia is a challenging condition that often requires sophisticated surgical approaches for abdominal wall reconstruction. The use of biological prostheses has shown to be a promising alternative in situations where the use of synthetic meshes is contraindicated due to complications such as infection or recurrence. This study reviews the impact of the use of biological prostheses in abdominal wall reconstruction in patients with complex incisional hernia, addressing aspects such as tissue integration, reduction in the rate of postoperative complications, efficacy in preventing hernia recurrence and the economic implications associated with these matrices. In addition, technological advances in the development of new biological matrices, barriers to their widespread adoption and the potential impact on patients' quality of life are discussed. Although biological prostheses have advantages in terms of biocompatibility and reduction of infectious complications, their high cost and variation in clinical outcomes require further studies to evaluate their comparative long-term efficacy and establish protocols for their use.
