This study analyzed the epidemiological evolution of congenital syphilis in Paraná between 2019 and 2024, focusing on the challenges of healthcare access and the effectiveness of public prevention policies. This descriptive and retrospective research used secondary data from DATASUS, SINAN, and SINASC to assess the disease's incidence, the profile of affected pregnant women and newborns, and regional disparities in reporting and treatment. The results revealed a significant increase in cases during certain periods, especially among vulnerable populations, such as young women with low educational attainment, and a low adherence to treatment by sexual partners. Despite advancements in testing and the control of vertical transmission, challenges remain regarding prenatal care coverage and the implementation of more effective screening and treatment strategies. The study highlights the need to improve maternal and child health policies, expand professional training, and strengthen epidemiological surveillance to ensure a sustainable reduction in congenital syphilis cases in the state.
