Introduction: Blepharoplasty, a surgical procedure designed to correct eyelid deformities, has gained prominence in aesthetics and medicine. This intervention aims not only to improve appearance but also to improve ocular function, especially in cases of excess skin that can obstruct vision. Although blepharoplasty is considered safe, like any surgery, it involves risks, including postoperative complications and dissatisfaction with the aesthetic results. Critical assessment of the associated benefits and risks is essential for informed decision-making by patients and health professionals. Objective: To assess the risks and benefits of blepharoplasty, providing a comprehensive view of the available evidence. Methodology: The methodology followed the PRISMA checklist, ensuring rigor in data selection and analysis. Searches were performed in the PubMed, Scielo and Web of Science databases, using five descriptors: "blepharoplasty", "complications", "aesthetic results", "patient satisfaction" and "recovery". Inclusion criteria consisted of peer-reviewed articles published in the last ten years, focusing on clinical studies and systematic reviews. Exclusion criteria included studies with small sample sizes, articles unrelated to the main topic, and publications that did not present original data. Results: Analysis of the articles revealed that the main benefits of blepharoplasty include improvements in facial aesthetics and visual function, with most patients reporting high satisfaction with the results. However, risks identified included bruising, infection, and aesthetic dissatisfaction. Recovery was generally rapid, but some patients reported postoperative discomfort. Conclusion: In summary, blepharoplasty offers a combination of aesthetic and functional benefits, but is not without risks. Careful assessment of each case, clear communication between physician and patient, and consideration of expectations are essential to optimize results and minimize complications. The review highlighted the importance of an informed and personalized approach to performing this procedure.
