The general objective of this study is to contribute to the safety of hypertensive patients in hospitals through the role of pharmacists in optimizing drug therapy and preventing risks associated with treatment. The methodology used was based on a bibliographic review of studies published between 2019 and 2024, available on Google Scholar. The search included scientific articles, systematic reviews, case studies, dissertations, and theses in Portuguese, focusing on the role of pharmacists in medication safety, medication reconciliation, prescription review, and education of hypertensive patients. The analysis was performed qualitatively using content analysis techniques. The conclusion showed that pharmacists play an essential role in the safety of hypertensive patients, mainly through medication reconciliation, prescription review, and education of patients and caregivers, thus reducing the risk of adverse events and promoting more effective therapeutic results.
