Introduction: Syphilis is a chronic infectious disease that can present cutaneous and systemic manifestations. Sexual transmission is the main form of transmission and can also occur through blood transfusion (rare currently) or accidents with contaminated sharps. Vertical, transplacental or hematogenous transmission accounts for cases of congenital syphilis, which is associated with high rates of neonatal morbidity and mortality. These elements configure gestational syphilis as a public health problem while highlighting the importance of adequate antenatal follow-up to prevent such outcomes. Aim: To describe the rate of congenital syphilis and the number of antenatal consultations that occurred in Brazil between the years 2017 and 2022. Methods: This will be an ecological study, therefore aggregate, observational, and transversal. Summary: In 2017, the incidence of congenital syphilis cases was 8.68 cases for every 1,000 live births. In the years 2018 to 2020, the values remained close, however, from 2021 an increase in incidence was found to 10.10 cases for every 1000 live births, among pregnant women who had at least one prenatal appointment. However, the highest incidence of congenital syphilis stands out within the group of patients whose mothers did not undergo any prenatal appointment. This value reached 84.56 cases for every 1,000 live births.
