Abstract

The objective of this study was to report the clinical experience of a psychology intern in applying Cognitive-Behavioral Therapy (CBT) in a case characterized by cognitive distortions, interpersonal conflicts, and narcissistic personality disorder traits. The methodology was based on an experience report derived from an internship in Psychotherapeutic Practices at the University of Vale do Itajaí’s School-Clinic, during which weekly psychotherapy sessions were documented, totaling nine sessions. The results highlighted significant challenges in the treatment, such as the patient's resistance to collaborating with the proposed interventions, as well as the identification of "all-or-nothing" thinking patterns, perfectionism, and dysfunctional beliefs about love and family. The patient’s abrupt termination of the therapeutic process limited the progress of the treatment. It is concluded that the case illustrates the relevance of the therapeutic alliance and a collaborative approach in Cognitive-Behavioral Therapy, especially in contexts where patient resistance and dysfunctional beliefs are predominant. The study reinforces the need for therapeutic strategies that directly address resistance and promote cognitive restructuring as essential elements for treatment effectiveness and the promotion of psychological well-being.
