HORMONAL REPLACEMENT THERAPY IN THE TREATMENT OF ERECTILE DYSFUNCTION: SYSTEMATIC REVIEW AND META-ANALYSIS

Erectile dysfunction (ED) is a common condition that affects the quality of life of many men around the world. Hormone replacement therapy (HRT), especially with testosterone, has been investigated as a therapeutic option for men with ED and low testosterone levels. In this systematic review and meta-analysis, 15 studies were analyzed, including randomized clinical trials and observational studies, that evaluated the effects of HRT in the treatment of ED. The results showed a significant improvement in erectile function in men undergoing HRT compared to those who received a placebo or were not treated hormonally. Analysis of randomized controlled trials revealed a standardized mean difference of 1.27 (95% CI: 0.87 to 1.67) in erectile function scores between the HRT and placebo groups. The success rate of sexual intercourse was also higher in the HRT groups. However, HRT was associated with an increased incidence of minor adverse events such as acne and sleep disturbances. The overall quality of the evidence was assessed as moderate, due to heterogeneity between studies and the potential for bias. In conclusion, HRT can be an effective therapeutic option for men with ED and low testosterone levels, but the decision to initiate treatment must be made with careful consideration of individual benefits and risks. More high-quality studies are needed to confirm these findings and investigate the long-term effects of HRT on erectile function and cardiovascular safety.
