Abstract

Pancreatic endocrine tumors (PTEs) are rare neoplasms that arise from pancreatic islet cells. Surgical management in the resection of these tumors is complex, given the diversity of clinical presentations and the need to preserve pancreatic function. In recent years, there have been significant advances in surgical techniques and perioperative management of patients with PTEs. Objective: The systematic literature review aimed to evaluate current practices and outcomes of surgical management in PTE resection, with a focus on identifying the most effective and safe techniques for patients. Methodology: The methodology was based on the PRISMA checklist, with research carried out in the PubMed, Scielo and Web of Science databases. The descriptors used were "pancreatic endocrine tumors", "surgical resection", "surgical management", "surgical techniques" and "perioperative management". Studies that reported resection techniques and clinical outcomes, published in the last ten years and available in full text, were included. Review articles, case reports and studies unrelated to the surgical management of PTEs were excluded. Results: 15 studies were selected. The results indicated that enucleation and distal pancreatectomy are the most common resection techniques. The selection of the surgical technique depended on the size and location of the tumor, as well as the presence of symptoms. Perioperative mortality was low, and most patients had significant improvement in symptoms after surgery. Conclusion: The conclusion of the review indicated that the surgical approach is the treatment of choice for resectable PTEs, with the surgical technique being adapted to the individual characteristics of the tumor and the patient. Adequate perioperative management and careful selection of patients for surgery were fundamental to the good outcomes observed.
