Introduction: The odontogenic keratocyst is conceptualized as a benign lesion originating from remnants of the dental lamina and is commonly found in the posterior part of the mandible. However, when it occurs in the anterior region of the mandible and exhibits multilocular radiographic characteristics, it may be mistakenly diagnosed as the botryoid variant of the lateral periodontal cyst. Objectives: The aim of this study is to describe the findings that differentiate an odontogenic keratocyst from a botryoid cyst. Methodology: This is an integrative literature review that, after the search, reading of titles and abstracts, and application of inclusion and exclusion criteria, selected 12 articles. Results and Discussion: Radiographically, the odontogenic keratocyst presents as a unilocular radiolucent lesion, typically associated with the crown of a tooth, which can complicate its diagnosis. On the other hand, the botryoid cyst is a multicystic variation of the lateral periodontal cyst, originating from the epithelial remnants of Malassez and reduced enamel epithelium, prevalent in men in the 5th decade of life, in the anterior region of the mandible, specifically in the canine and premolar regions. Conclusion: Clinical findings provide clues for diagnosis, but the combination of histopathological findings with the results of different radiographic imaging exams is essential for the definitive resolution of the case.
