In a hospital environment, it is common in clinical practice to use different medications simultaneously, which increases the risk of potential drug interactions, which can lead to a failure in therapeutic treatment and even an increase in hospital costs, causing the patient need to remain hospitalized for longer. The objective of this study is to explain the potential drug interactions that may occur in hospital prescriptions for hospitalized patients, raise awareness and explain the importance that hospital pharmacovigilance has in detecting, evaluating, monitoring and reporting drug-related problems, and even preventing them. . The relevance of the clinical pharmacist in the hospital's multidisciplinary team, to identify and resolve problems related to medications and generate even greater safety during patient treatment. And show the strategies used today to reduce medication dispensing errors in hospital pharmacy.
