Objective: to describe the distribution of AIDS cases recorded in Maranhão from 2022 to 2025 by year, sex, age group, and exposure category. Methods: a descriptive retrospective epidemiological study using public TabNet/DATASUS data. Absolute and relative frequencies were calculated; 2025 records were preliminary and available through September 30. Results: 4,958 cases were counted: 1,334 in 2022, 1,377 in 2023, 1,280 in 2024, and 967 in 2025. Men accounted for 70.9%; the 20-34 and 35-49 age groups accounted for 38.1% and 37.5%, respectively. Heterosexual exposure represented 52.9% of all cases and 64.2% of records with known information; 17.6% had an unknown category. Conclusion: cases were concentrated among men and adults aged 20-49 years. Preliminary 2025 data and incomplete exposure information require cautious interpretation and improved surveillance.
