Small cell neuroendocrine carcinoma of the cervix (SCNECC) is an extremely rare, high-grade neuroendocrine gynecologic tumor with an aggressive behavior, associated with a high recurrence rate and poor prognosis. We present the case of a 37-year-old multiparous woman and smoker who presented with postcoital bleeding. Physical examination revealed a 6-cm exophytic cervical mass. The initial biopsy established a differential diagnosis between basaloid squamous cell carcinoma and SCNECC. Immunohistochemistry confirmed the diagnosis (p40 negative, synaptophysin positive). Imaging studies and clinical staging under anesthesia determined FIGO stage IIIC1 (due to parametrial and right iliac involvement). The patient was referred to medical and radiation oncology for multimodal treatment.
